Primary Supervision Opt Out Forms – 2012/2013 School Year

Please note that the Primary supervision opt out form in respect of the 2012/2013 school year has been placed on the Department’s website. The form can be accessed by clicking on the link below or on the DES website as follows: Schools and Colleges -> Services -> Supervision with the heading Teachers who have opted out of supervision duties.
The form should be completed and returned by Friday, 25 January 2013 to the: Primary Teachers Payroll Section, Department of Education & Skills, Cornamaddy, Athlone, Co. Westmeath.
